Prince George Ball Hockey League (PGBHL) — Waiver and Release of Liability
By registering to participate in the Prince George Ball Hockey League (PGBHL), I understand that
ball hockey is a physical sport that involves inherent risks, including but not limited to collisions,
falls, contact with other players and equipment, and risks associated with facilities and playing
surfaces. These risks could result in injury, illness, permanent disability, or death.
I knowingly and voluntarily choose to participate and assume all such risks.
I hereby release, discharge, and hold harmless the PGBHL, its directors, officers,
organizers, volunteers, scorekeepers, officials, and sponsors, as well as the Roll-A-Dome
venue, its owners, employees, and agents, from any and all claims, liabilities, demands, or causes of
action arising out of my participation, whether caused by negligence or otherwise, to the fullest extent
permitted by law.
I certify that I am physically fit to participate; I will follow all rules, policies, and directions of
league officials and facility staff; and I will use league-approved equipment in good condition. I accept
responsibility for my conduct on and off the playing surface.
I consent to the use of my name, image, or likeness in league communications, statistics, and media
without compensation. If I am registering a minor, I confirm that I am their parent or legal guardian and I
accept this waiver on their behalf.

I am waiving certain legal rights, including the right to sue.
If under 18 years of age, a parent or legal guardian must accept this waiver.
